Currently serving eight (8) counties in the Eastern Shore region(Dorchester, Talbot, Queen Annes, Caroline, Kent, Wicomico, Worcester and Somerset).

This program uses a trauma-informed, culturally responsive, veteran-informed model of care designed to meet the unique needs of military-connected individuals and families. Services focus on early identification of behavioral health needs, timely connection to appropriate care, reducing barriers to treatment, strengthening continuity of care, and supporting long-term stability and wellness.

Many veterans and military-connected families face challenges accessing
behavioral health services, especially in rural communities.
Without early screening and coordinated support, behavioral health concerns such as PTSD, depression, anxiety, and substance use disorders often go unaddressed until they reach crisis levels. This program helps identify needs early and connect individuals to the right support.
