Eastern Shore Behavioral Health
Screening and Care Coordination Program (Vets Program)

Currently serving eight (8) counties in the Eastern Shore region(Dorchester, Talbot, Queen Annes, Caroline, Kent, Wicomico, Worcester and Somerset).

Locations: 

This program uses a trauma-informed, culturally responsive, veteran-informed model of care designed to meet the unique needs of military-connected individuals and families. Services focus on early identification of behavioral health needs, timely connection to appropriate care, reducing barriers to treatment, strengthening continuity of care, and supporting long-term stability and wellness.

Without early screening and coordinated support, behavioral health concerns such as PTSD, depression, anxiety, and substance use disorders often go unaddressed until they reach crisis levels. This program helps identify needs early and connect individuals to the right support.

Participants receive education and support to better understand behavioral health conditions and available resources. Topics may include:

  • Connecting participants with behavioral health providers
  • Coordinating care with medical and community providers
  • Assistance navigating treatment options
  • Follow-up after emergency or inpatient care
  • Support during transitions between services
  • Accessing additional resources
  • Transportation assistance for appointments
  • Help scheduling services
  • Appointment reminders
  • Accompaniment to community resources when needed
