Our Vision

Organizational Origins and Development

Wraparound Maryland traces its roots to two founding agencies—Transitions and New Transitions. Our evolution has been guided by a shared commitment to advancing a care coordination process that aligns with the core philosophies of the System of Care framework.
This journey began with the establishment of New Transitions, founded by Kimberly Cooke, who now serves as the Executive Director of Wraparound Maryland, Inc. Ms. Cooke was first introduced to the Wraparound model while working as a mental health professional in her hometown of Salisbury, Maryland. At the time, the model was gaining national attention as a progressive alternative to traditional service approaches. Her involvement began when a family was asked whom they trusted to provide support, and they identified our organization—citing a sense of being heard and valued.

Recognizing the potential of the model and its impact on the families she served, Ms. Cooke made the pivotal decision to leave her position and establish New Transitions, with the mission of becoming a care coordination agency. The organization quickly gained national recognition for its success in implementing the Wraparound model and achieving positive outcomes for families.
At New Transitions, Ms. Cooke brought together a team of like-minded professionals dedicated to supporting youth and families with complex needs. The organization emphasized a strengths-based approach and operated within the local communities of Wicomico County. Early on, the team identified care coordination—through the Wraparound model—as a powerful tool to support families often left feeling frustrated and overlooked by conventional service systems.

In collaboration with local child-serving agencies, community partners, and stakeholders, New Transitions played a central role in advancing Systems of Care reform in Wicomico County. The organization actively participated in training and coaching forums led by national experts, including Sheila Pires, author of the System of Care Primer, and The Institute for Innovation and Implementation (formerly the Innovations Institute). These opportunities helped to deepen local expertise, cultivate Wraparound champions, and accelerate the spread of System of Care practices throughout Maryland.

In 2008, the State of Maryland recognized New Transitions for its leadership in implementing the Wraparound model and promoting System of Care principles. As a result, New Transitions was selected as one of four pilot sites to implement care coordination across the state. During this pilot phase, New Transitions effectively served Wicomico County as a Care Coordination Entity, laying the groundwork for Maryland’s statewide adoption of the Care Management Entity (CME) model.

This success led the Governor’s Office for Children—supported by the Children’s Cabinet—to establish a broader statewide System of Care initiative and solicit proposals from organizations capable of serving as CMEs. In response, Wraparound Maryland, Inc. was founded as a 501(c)(3) nonprofit organization. In 2009, Wraparound Maryland was awarded two of the state’s three regional CME jurisdictions: Baltimore City and the Southeastern Region, marking a significant milestone in expanding care coordination across Maryland.
