Providing One-On-One Support to Foster Parents and Families
Overview of Services
ONE-ON-ONE SUPPORT
Provide one on one support to foster parents and family to address concerns and prevent placement disruptions through the development of Individualized Support Plans (ISP).
Development of individualized support plans are designed to:
- Strengthen parenting skills.
- Improve relationships with youth.
- Build partnerships with biological families as appropriate.
- Develop community connections and natural supports.
- Coordinate services (if multiple agencies and/or services are in place).
- Psychoeducational, development, and emotional support.
- Modeling of parenting skills and crisis intervention techniques.
- Problem-solving around barriers to maintaining the youth such as supervision, childcare, or mental health concerns.
- Stress management and overall support of parents may be included in the plans.
TRANSITION SUPPORT
PLANNING FOR TRANSITION
This service is designed to provide consultative planning for the transition of youth from a congregate care setting into a foster home in effort to improve placement stability outcomes by proactively addressing concerns related to the transition. The goal of this service is to proactively plan around the identified youth and family’s support needs through efforts including, but not limited to:
- Creation of individualized transition support plan.
- Provide support to the families to improve placement stability and likelihood of positive transition by identifying and proactively planning around potential challenges and concerns.
- Provide ongoing support to celebrate progress and address challenges to placement.
- Create crisis support plans designed to proactively address crisis as they arrive.
- Communication with all parties involved to help support the youth and family’s team to during times of transition.
